Beta Tester 

I would like to be a beta tester for the Make Fit Simple App and choose to participate at my own free will and choice. No obligation is required just an incentive offered for participating in testing. 

As a beta tester I agree to the following guidelines to receive the incentive: 

• DFH will provide an incentive of $100 to Tester for participation as a beta tester. 
• This incentive will be disbursed upon completion of testing and feedback for six required App Update Versions (each version may include multiple builds to test until the version is accurate for release to the public) that will span across the year.  Incentive payment will be through Venmo or Zelle.
• Tester acknowledges and agrees that participation in the beta testing program is voluntary. 
• Tester agrees to maintain confidentiality regarding any pre-release information or materials provided by DFH.

There are no obligations for continuing as a beta tester, you can opt out at anytime. If you do not participate, the incentive is nullified.
